Why are we coordinating this Collaborative?
To improve access to affordable patient-centered primary care for those most vulnerable
What are the objectives?
- 2014 Primary Care Access Plan: develope a collaborative plan for providing timely access to patient-centered primary care services in Central Ohio - primarily for low-income residents eligable for health insurance coverage in 2014 via federal health care reform legislation
- Optimizing Prescription Drug Utilization: develop a collaborative plan for optimizing the utilization of affordable prescription drugs via the federal 340B program
Intro:
Central Ohio has three Federally Qualified Health Center (FQHC) organizations. FQHCs in our community are independent non-profit organizations that provide primary health care at eight health center locations. FQHCs are safety-net providers of cost-effective, high-quality primary and preventative health care for vulnerable individuals and families.
