| Intro: | The fragmentation of our delivery system is a fundamental contributor to the poor overall performance of the U.S. health care system. Poor communication and lack of clear accountability for a patient among multiple providers lead to medical errors, waste, and duplication. |
| Purpose: | To improve patient-centered care coordination across emergency departments and with community health centers for all people in central Ohio – starting with hospitals located in Franklin County. |

Charitable Pharmacy |

| Intro: | Access HealthColumbus conducted the initial research on establishing a charitable pharmacy in central Ohio. Our collaborative approach led to the formation of the Charitable Pharmacy of Central Ohio under the leadership of the West Ohio Conference of the United Methodist Church. Moving forward, we will continue to assist the Charitable Pharmacy of Central Ohio’s board and staff with efforts to expand access to affordable prescription drugs. |
| Purpose: | To create a “bridge” pharmacy to increase access to affordable prescription drugs for low income, uninsured and underinsured individuals who experience gaps with insurance coverage and/or challenges in accessing medications through other available programs. |

Federally qualified health center collaborative |

| Intro: | Our community has three Federally Qualified Health Center (FQHC) organizations that provide primary health care via eight health center locations.
FQHCs, independent non-profit organizations, aresafety net providers of cost-effective, high-quality primary and preventive health. |
| Purpose: | To promote collaborative efforts across FQHCs that improve the efficiency and effectiveness of these organizations – leading to expanded access to affordable primary health care for vulnerable people. Access HealthColumbus is assisting FQHC leadership with the following:

Latino HEalth Connector |

| Intro: | In 2008 and 2009, Access HealthColumbus received a national Robert Wood Johnson Foundation grant to coordinate a two-year demonstration project focused on improving access to health care for employed Latinos in Franklin County. Based on the outcomes and results from the demonstration project, a collaborative emerged in March 2010 to advance the Latino Health Connector in our community.
